To request access to information, you will need to:
- Determine whether you are applying for personal or non-personal information.
- Complete and submit a Right to Information and Information Privacy Access Application form (PDF). Please note that the CCC is currently unable to accept online applications for access.
- Ensure that you provide sufficient details
- about the documents or information you are requesting to enable the CCC to identify the relevant documents
- of an address to which the CCC can send notices.
- Provide proof of identity if you are seeking personal information.
- Include payment of the application fee (unless you are seeking personal information only).
Applying to access personal or non-personal information
- Personal information only
If you are applying only for your personal information, your application is considered under Information Privacy (IP) provisions, and there is no application fee. However, you will need to provide proof of your identity.
- Both personal information and non-personal information
If you are applying for both personal and non-personal information, your application is considered under Right to Information (RTI) provisions, and an application fee applies. You will also need to provide proof of your identity.
- Non-personal information only
If you are applying only for non-personal information, your application is considered under Right to Information (RTI) provisions, and an application fee applies. No proof of identity is required.
- Applying on someone else’s behalf
If you are applying for access to information on somebody else’s behalf, you still need to indicate what kind of information you are seeking. Both you and the person you are representing must provide proof of your identity.
