About this form

This notification meets a duty to notify the Crime and Corruption Commission of corrupt conduct under the
Crime and Corruption Act 2001.

Who can use this form?

This form is used to notify corrupt conduct on behalf of a Queensland public sector agency. If you are making a complaint as an individual and not on behalf of your agency, please visit the members of the public report corruption form.

  • Unit of public administration Chief Executive, delegate, liaison officer or their authorised representative
  • Other agencies referring a matter to the CCC

Who can’t use this form?

Please visit the report corruption form if you are a:

  • Member of the public
  • Public sector employee making a complaint as a private citizen

What do I need before I notify?

You must notify the CCC if you reasonably suspect corrupt conduct has occurred. You are not required to conduct any preliminary inquiries, nor do you need evidence to any standard.

Acknowledgement

The CCC has the authority to act as we deem necessary in the assessment of this notification. This may include:

  • Passing on any information to relevant public sector agency
  • Contacting witnesses
  • Conducting any other inquiries necessary to deal with the notification
